Form for days off
If you want a day off [outside the summer holiday], please fill in the form below. The form will open your email program, and you can send it.
Requests made more than 6 weeks in advance will not (yet) receive approval. You may submit a request for a day off that starts 6 weeks or later, so the scheduler can take it into account.
You can only assume approval if you have either received an email from me or if you see 'vrij' (off) in a green box on the schedule for the desired date.
